Attribution for Deceased Authors

Can anyone tell me whether there is a convention governing
authors who have died between submission of a paper and publication? Do you
describe them as 'formerly of....(the address given when the paper was
submitted)' or put simply put 'deceased' against their name?

Psychogeriatrics, when this happened, we put an asterisk and then a
footnote saying that the author had died on a particular date.
