Review Form

We are going to revise our review form. I have searched and found some, but to have a well-designed form, we need to find more. May I ask you to kindly send me a copy of your journal's review form as an e-mail attachment?

From the viewpoint of a reviewer...

Wearing my reviewer hat (as I have a couple times this week), I'm commenting on a trend I've observed (with an admittedly small n):

Decreasingly grateful communications to reviewers. As we move to the much-more-convenient-for-editorial-staff automated review systems, we should be vigilant about inconveniencing reviewers. Do I really have to give you my full mailing address before I can give you a good review? Some systems won't let you even look at the ms until you've "completed all fields".

Mind you, I'll review anything on physician health anyone sends me, but I do generally get treated much more nicely by people who pay me to do this kind of work for them than by those who ask me to volunteer. With exponentially growing #s of mss, this can represent considerable work for some reviewers, and we should be super-nice to these least-rewarded, but essential participants in our publication process.
